What is needed to apply for the vaccination program in Nigeria

Filling the vaccination form, you need your basic information such as:

  • full name,
  • phone number,
  • email address,
  • date of birth,
  • sex,
  • a form of ID,
  • a residential address including states,
  • LGA of residence, and
  • ward of residence. 

…including to select your preferred vaccination date and time slot and upload a photograph.

The form asks if you work in the health sector and if you have any medical conditions or allergies. 

Finally, a code is automatically generated, which you’ll be required to type in the code (as it is displayed on your screen) and this completes your registration process. 

You should receive a message that your registration has been completed. You will also be issued a vaccination ID. The message goes thus: 

“Registration Completed!  Your Vaccination ID is:

Your data is successfully received.

Thank you for taking the time to fill the form, you will receive an SMS and email shortly containing your Vaccination No. and other instructions.”

The National Primary Health Care Development Agency (NPHCDA) had two aims of providing the strategy on board;

1. To captures an individual’s data and giving them the ability to schedules for COVID 19 vaccination. This means individual’s who applies for the vaccine online will have the opportunity to fix the date when its convenient to get the covid-19 vaccination administered.

The registration is simple, and straight forward. However, you can use either a mobile phone, tablet, or PC access the Covid-19 Vaccination registration Online Form and complete the registration process.
